Effective communication and coordination are essential for optimizing transitions of care in patients with venous thromboembolism (VTE). This interactive, case-based session will explore key challenges across the continuum of VTE management—from initial diagnosis and hospitalization to outpatient follow-up, long-term care, and emergency situations. Through case scenarios, faculty will highlight best practices for patient education, anticoagulation management, and care team collaboration. Attendees will engage in polling and discussions to assess current practices, identify barriers, and apply evidence-based strategies to improve patient outcomes.
